Infectious Disease Flashcards
pink foamy exudate in alveoli
pneumocystis jiroveci pneumonia
BAL = legally drunk
80 mg/dl
BAL cuases drowsiness? stupor?
drowsiness = 200 and stupor = 300
microscopic pathology seen with influenza infection
lymphoctes + necrosis + squamous metaplasia + alveolar edema + hyaline membranes +/- fibrinopurulen alveolar discharge (if superimposed bacterial infection)
why does flu make people susceptible to pneumonia?
flu kills respiratory epithelium with KO the mucociliary escalator
grocott stain + coin lesions
histo
simultaneous nuclear and cytoplasmic inclusions
CMV
presentations/diseases that CMV can cause in a person wtih AIDS
enteritis, retinitis, disseminated disease
MC clinical manifestation of CMV in immunocompetent hosts
~ mono w/ abnormal LFTs and hepatomegaly and LAD
3 distinc forms of aspergillosis
- colonizing = ball that fills cavity
- allergic = bronchopulmonary + eosinophillia
- invasive = immunocompromised starts in lungs and invades vessels –> hemorrhage, infarct or go to brain
fruiting body seen in aspergillus
conidiophore
MC pathogenic parasite in humans
giardia
is giardia invasive
no
characteristic shape of giardia
pear shaped
diff diagnosis for anemia in a mexican immigrant
anemia of chronic disease, Fe def, folate or B12 def, malaria, myelodysplastic syndrome, autoimmune hemolytic anemia, aplastic anemia
diff diagnosis of peripheral nodular lung infiltrates
TB, histo, sarcoid, S. aureus, Pneumococcus, granulomatosis with polyangitis, mets
MC helminth infection in the world
ascariasis
where do ascariasis typically live
small intestine
life cycle of ascariasis
eggs develop into larvae in soil. human eat soil. larvae penetrate intestine, enter portal system and go to liver, heart, and lungs. develop some in lungs, ride mucocilialry ladder up to pharynx, and then enter esophagus to get to intestine where they mature into adult worms
loeffler’s syndrome = ? and is assc with
eosinophillia + cough + transient pulm infiltrates + intestinal obstruction + biliary obstruction + liver abcess + pacreatitis + appendicitis = ascarasis
what is necessary for eosinophillia to develop in a pt with ascariasis
larval migration/penetration thru intestine
Dx of ascariasis
eggs in stool
treatment of ascariasis
albendazole or mebendazole
2nd MC parasitic worm in worth
Trichuriasis (whipworm)
