Infection CPC Flashcards
Widespread, bilateral ground glass shadowing with reduced exercise tolerance
PCP pneumonia
Tx of PCP pneumonia
Co-trimoxazole
PCP staining
Silver stain - grocott gromori
What is a side effect of co-triimoxazole?
Widespread erythematous rash
Risk of diff infections as CD4 count falls progressively
Link between type of immunodeficiency and type of infection
Who does actinomyces affect? Why is it an issue?
Alcoholics and immunocompromised patients
Indolent and slow growing - difficult to treat
What type of bacteria is actinomyces? Which other bacteria is it associated with?
Gram positive rod
Branching
**closely associated with nocardia- another gram positive rod
Classic histological staining pattern of actinomyces
Basophilic sulfur granules
**need to specifically request for this test
What infections does actinomyces cause?
Classically causes lung and brain abscesses
Why do prosthetic joint infections need surgical debridement?
Because bacteria can form biofilm making it difficult for antibiotics to penetrate so require surgical debridement
What is the most important aspect of treating prosthetic joint infections?
Often need surgical debridement to prevent the infection from spreading
i.e. often antibiotics are not enough
Treatment of c difficile infection
Non-severe
- Metronidazole
- If not responding within 72h–> switch to vancomycin
Severe
- Vancomycin +/- metronidazole
Which ribotype of C difficile is harder to treat?
RIbotype 027
was assoictaed with an outbreak in june 2005
WCC and CRP in c difficile colitis
High WCC
Normal CRP
