Propofol MOA
GABA agonist; selectively modulates GABA(a) receptors
Increases: CHLORIDE conductance
Hyperpolarization of the postsynaptic cell membrane and functional inhibition of the postsynaptic neuron
Etomidate MOA
Selective modulator of GABAa receptors (need GABA present)
Only carboxylated imidazole-containing *No analgesia
Ketamine MOA
non-competitively binds to the phencyclidine site inside the NMDA receptor and blocks the influx of calcium, resulting in a decrease in the presynaptic release of glutamate
Weak GABA a receptor action
Other receptor sites: opioid (μ, δ, and κ; weak σ)
Propofol Pharmacodynamics
Metabolism: 60% Hepatic, 40% Renal, Pulmonary (clearance)?
Other Considerations: egg yolk allergy (lecithin), caution in dec EF & hypotension
Ketamine Pharmacodynamics
Metabolism: Hepatic, cytochrome P450 enzymes
Other Considerations: Emergence delirium, mAChR antagonist (secretions), Increases ICP, & Caution if catecholamine depletion
