ILD- Broncheoalveolar Lavage Flashcards
CD4:CD8 ratio > 3.5 most specific diagnosis; lymphocytosis
Sarcoidosis
Hemosiderin-laden macrophages, red blood cells
Diffuse alveolar bleeding
Marked lymohocytosis > 50%
Hypersensitivity pneumonitis
Atypical hyperplastic type II pneumocytes
Diffuse alveolar damage
Drug Toxicity
Pneumocystis carinii, fungi, CMV transformed cells
Opportunistic infections
Milky effluent, foamy macrophages and lipoproteinaceous intraalveolar material (periodic acid schiff stain- positive)
Alveolar proteinosis
Increased CD1+ Langerhans cells, electron microscopy demonstrating Birbeck granule in lavaged macrophage (expensive and difficult to perform)
Pulmonary Langerhans cell histiocytosis
Dust particles by polarized light microscopy
Silicosis
Accumulation of specific lipopigment in alveolar macrophages
Lipoidosis
Foamy macrophages, mixed pattern of increased cells characteristic, decreased CD4:CD8 ratio
Organizing pneumonia
Eosinophils > 25%
Eosinophilic Lung Disease
Fat globules in macrophages
Lipoid pneumonia
Dust particles, ferruginous bodies
Asbestos related pulmonary disease
Malignant cells
Lymphangitic carcinomatosis
Alveolar cell carcinoma
Pulmonary lymphoma
