What are the visual impairment guidelines to qualify for an IEP?
- visual impairment, not primarily perceptual in nature, resulting in a measured visual acuity of 20/70 or poorer in the better eye with correction
OR - A physical eye condition that affects visual functioning to the extent that
special education placement, materials and/or services are required in
educational setting
What is included in the typical 504 Program or Plan?
- Specific accommodations, supports or services for the child
- Names of who will provide each service
- Name of the person responsible for ensuring the plan is implemented
What is the step that allows the school district to pinpoint whether your child has a disability that will require special education services? 1. How long do they have to complete it? 2.

What is included in the IEP Program or Plan?
- present levels of academic and functional
- Annual education goals for the child and how will track progress
- services the child will get
- timing of services – when they start, how often they occur and how long they last
- Any accommodations
- Any modifications – changes to what the child is expected to learn or know
- How the child will participate in standardized tests
- How the child will be included in general education classes and school activities
Who initiates IEPs and 504s?
- parent
- school staff member
- Ohio department of education or another public agency
What does the Evaluation Team Report include?
- Statement about why child was referred for an evaluation
- Background information about the child’s social and academic history
- A list of tests performed and the records reviewed
- Summary of test results and scores
- Explanation of what the test results mean
What vision test is used for the fine motor evaluation?
Beery VMI or TVPS
What does the evaluation consist of?
- psychological evaluation
- interviews
- physical exam
- academic testing
- functional behavior assessment
Who is part of the team for an IEP?
- child’s parent
- child’s teacher
- at least one special ed teacher
- A representative the school who has knowledge of the child and/or academic requirements of the general education curriculum as well as interpret evaluation results (psychologist or therapist)

What are good questions to ask during a child’s exam?
- Ask about school performance
- Ask about favorite subjects
- Do you like to read?
- Does your child avoid near work and reading?
- Is your child at proper reading levels?
- Specific questions about symptoms

What does the Eye Exam Requirement Amendment say?
Requires an eye exam to be completed within 90 days of initiation of IEP
