idiopathic inflammatory myocitis-infiltration of the proximal muscle by cells
deramtomyosistis-skin +mucle
Polymyosistis-only muscle
Signs and Sx of idiopathic inflammatory myelitis
Dermatomyositis-Heliotrope rash-purple rounder the eyes-upper eyelids
Gottrons papuples-erythrema over knuckles
shawl sign-looks like sunburn in shawl patter
Mechanics hands-
reynauds
Difficulties moving/doing mechanical moves
muscle weakness and atrophy
fatigue, weight loss
Investigations of idiopathic inflammatory myelitis
Look at muscle
CK-super raised (few thousands)
ANA-sometimes hit or miss
MRI scan-can see oedema in muscle
EMG-abnormal function
confirm with muscle biopsy
Management of idiopathic inflammatory myelitis
acute flares- steroids +/- IVIG
Ongoing- oral steroids 1st line
azathioprine/methotrexate 2nd
+ always do a malignancy screen as often caused by cancers
Aetiology and risk factor of idiopathic inflammatory myelitis
Aetiology is unknown
But thought to be genetic, environment, and causative factors (EBV)
OFEN LINKED WITH CANCERS
risk factors:
Children and under 40 FHX Black Female White male exposure to sun
Epidiemology of idiopathic inflammatory myelitis
rare -5.5 in 1 million in US
