What is fibromyalgia
Chronic pain syndrome classified by presence of widespread body pain with no specific cause, at least 3m
and at least 11/18 designanted tender points
RF- family hx, rheum conditions
female
Sx and signs of fibromyalgia
Usually neuropathic type pains (numbness/tingle/gnawing)
Fatigue, brain fog
sleeping issues
Stiffness
physical- 11/18 pointss around the body (done by specialist)
purely clinical exam
all ix are normal (CRP, CK, etc)
worth cheching ddx- RA, SLE, AS, PA etc
Management of Fibromyalgia
1st line TCA
amitryptiline/- about 30% get beter
SSRI (duloxetine- good if comorbid depress
Gabapentinous- comordbid sleep issue
+ALL pt should have education, exercise, CBT
+ analgesics -NSAIDS
