How to treat coag-neg Staph CLABSI

A
  • 5-7 days abx if catheter is removed
  • 10-14 days in combination with antibiotic lock therapy is catheter is not removed
  • in patients without prosthetic devices or endovascular hardware, may be treated by removing catheter and getting repeat BCx

(coag-neg Staph is less virulent than Staph a and less likely to cause metastatic infection)

How long do S. aureus CLABSIs need to be treated.

A
  • up to 4-6 weeks
  • 14 day courses can be considered in patients whose catheter was removed and do not have DM, immunosuppression, implanted prosthetic devices, and no evidence of endocarditis by TTE/TEE AND no evidence of infected thrombophlebitis whose fever and bacteremia resolve within 72 hours and an exam negative for any evidence of metastatic infection
