RASS SCORE
denotes level of sedation ranging from -5 to +4 (un-rousable to combative) and can also identify safety concerns or barriers to mobilisation.
Dejonhe Criteria
- open/close your eyes
- Look at me
- poke out your tongue
- nod your head
- raise your eyebrows
minimum of 3/5 is deemed for the pt to be awake
PFIT
is one ICU specific outcome measure which assesses isolate strength, conditioning as well as functional status. (/12)
includes STS, MOS and upper/lower limb strength testing
Indications for a tracheotomy
- prolonged mechanical ventilation
- unable to expectorate copious secretions
- unable to protect upper airways
- upper airway obstruction
- protect from aspiration
ready for decannulation?
- ideally, alert and responsiveness
- tolerating cuff deflation
- stable o2
- not requiring mechanical ventilation
- no new CXR changes
- able to manage secretions and cough

Mechanical Ventilation is a method to mechanically assist or replace spontaneous breathing.
- Positive Pressure Ventilation: gas is pushed into the trachea/lungs
- Negative Pressure: gas is essentially sucked into the lungs
