What are the most common presentations of GERD?
Pyrosis, sourbrash, belching, bloating, dysphagia, halitosis, chronic cough, chronic laryngitis
What are risk factors for GERD?
BMI, Alcohol, Caffeine, Pregnancy, Hiatal hernia, Chocolate, Peppermint, NSAIDs
Why are pregnant women at increased risk for GERD?
Lower esophageal sphincter tone, hiatal hernia and decreased peristalsis.
What observational findings are indicative of GERD?
Loss of tooth enamel, hoarseness
What do you absolutely not want to miss in a patient presenting with classic GERD symptoms?
Coronary artery disease (unstable angina), aortic dissection, esophageal cancer
What lifestyle changes help manage GERD?
Elevate head of bed, weight loss and avoid known triggers
What medications are used to treat GERD?
Antacids, H2 blockers, PPIs
Medications with tachyphylaxis
H2 blockers. They stop working over time, but work again after taking a break from using them.
What are indications for endoscopy in patients with GERD?
Dysphagia, odynophagia, bleeding, anemia, unexplained weight loss, not responsive to PPIs
What are indications for surgery in patients with GERD?
Medication tolerance
