hypertension in pregnancy Flashcards
hypertension in pregnancy - types
- Gestational hypertensiocn (pregnancy-induced hypertension)
- Preeclampsia
- Eclampsia
- HELLP syndrome
Gestational hypertension (pregnancy-induced hypertension) - characteristics
- BP more than 140/90 after 20th week of gestation
- No pre-existing hypertension
- No proteinuria or end-organ damage
Gestational hypertension (pregnancy-induced hypertension) - drugs (short mechanism of action) and management
- hydralazine (vasodilator by cGMP increasing)
- labetolol (alpha-beta blocker)
- α-methyldopa (a-2 blocker)
- nifedipine (dihydropiridine Ca2+ channel blocker)
- deliver at 37-39 weeks
Preeclampsia - definition
New-onset hypertension with erther proteinuria or end-organ dysfunction after 20th of gestatation
Preeclampsia may preceed to
eclampsia (+ seizures) and/or HELLP syndrome
Preeclampsia but before 20 weeks?
hydatidiform mole
Preeclampsia is caused by
abnormal placental spiral arteries –> endothelial dysfunction, vasoconstriction, ischemia
Preeclampsia - high incidence in …
- pre-existing hypertension
- diabetes
- chronic renal disease
- autoimmune disorder
Preeclampsia - complications
- placental abruption
- coagulopathy
- renal failure
- uteroplacental insufficiency
- eclpampsia
Preeclampsia - treatment
- anithypertensive
- IV magnesium sulfate (to prevent seizures)
- definitive is delivery of fetus
uteroplacental vascular insufficiency??
(or Placental insufficiency): placenta is unable to deliver adequate supply fetus
Eclampsia?
Preeclampsia + maternal seizures
Eclampsia - complications
maternal death due to:
- stroke
- intracranial hemorrhage
- ARDS
Eclampsia - treatment
- IV magnesium sulfate
- antihypertensives
- immediate delivery
HELLP syndrome
HELLP: 1. Hemolysis 2. Elevated Liver enzymes 3. Low Platelets a manifestations of severe preeclampsia
