What is stage 1 Hypertension
140/90 to 159/99 (normal)
135/85 to 149/94 (ambulatory)
What is stage 2 Hypertension
160/80 to 180/120 (normal)
> 150/95 (ambulatory)
What is stage 3 Hypertension
> 180/120
When should you offer pharmacological treatment
Stage 1 and…
- < 80years with kidney disease
- < 60years with QRISK 10
- >80 with BP over 150/90
1st and 2nd Line for Hypertension in <55 years or Type 2 diabetes
1st Line:
- ACEi / ARB
2nd Line:
- ACEi / ARB + CCB / Thiazide
1st and 2nd Line for Hypertension in >55 years or Afro-Caribbean
1st Line:
CCB
2nd Line:
CCB + ACEi / ARB / Thiazide
3rd Line Hypertension for all patients
3rd Line:
Triple Therapy
ACEi / ARB + CCB + Thiazide
4th Line Hypertension for all patients
If serum Potassium is >4.5
ADD: BB or Alpha Blocker
If serum Potassium is <4.5
ADD: Spironolactone
ACEi / ARB Side effects
- Cough (ACEi only)
- Angioedema (ACEi only)
- Hyperkalemia
- Renal Failure
- Hepatic Failure
- Dizziness
What drugs interact with ACEi / ARB to cause renal failure
- Potassium Sparing Diuretics
- NSAIDs
What drugs interact with ACEi / ARB to increase hyperkalaemia
- Potassium Sparing Diuretics
- NSAIDs
- Heparins
- Beta Blockers
