What is the definition of systemic hyperglycemia?
BGL greater than 300, with signs of severe dehydration, AMS, and/or shock.
Prehospital care should be focused:
around the treatment of severe dehydration and support of vital functions
Walk through the protocols BLS to ALS:
DRABCDE, oxygen, consider possible unconciousness causes, BGL, establish IV, normal saline 10ml/kg infusion over 1 hr. Nasal capnography, 12 lead, reassess blood glucose at arrival
What should you give when BGL is over 300 and signs of dehydration are present?
Infusion of normal saline 10 ml/kg over 1 hr.
What are the signs and symptoms of hyperglycemia?
anxiety, agitation, confusion, dry, red, warm skin, acetone smell on breath, kussmaul respirations, dry mouth and intensive thirst, abnormal or hostile behavior, tachycardia, dizziness/headache
Differentials for hyperglycemia
hypoxia, stroke, brain trauma, alcohol intoxication, toxin/substance abuse, medication effect/overdose.
