Normal range for phosphate?
Normal Range: 1.9 - 4.3 mg/dL
Importance of phosphate?
muscle contraction
When do you experience symptoms of phosphate deficiency?
develop when phosphate is below 0.6mmol/L, non specific
Differentials of low phosphate?
Hypokalemia, Hypothyroidism, Hypomagnesemia, Primary muscle disorder
What other electrolytes do you test for in phosphate deficiency?
- calcium
- magnesium
- potassium
Calcium and phosphate deficiency?
- High calcium levels with low phosphate levels suggest primary hyperparathyroidism
- low calcium levels suggests vitamin D deficiency or malabsorption
Magnesium and phosphate deficiency?
Low magnesium levels are also suggestive of poor nutrition
Potassium and phosphate deficiency?
Serum potassium derangements, especially hypokalemia, may occur with such as diabetic ketoacidosis and alcoholism
Causes of hypophosphotaemia?
- internal redistribution
- decreased intestinal redistribution
- increased urinary excretion
Causes of Internal redistribution in hypo?
- hyperinsulinaemia
- Acute respiratory alkalosis
Trauma, burns, sepsis
symptoms - tetany
Causes Decreased intestinal redistribution in hypo?
- inadequate intake - alcoholism
- antacids containing aluminium or magnesium
- steatorrhea
- chronic diarrhea
symptoms - respiratory failure
- hypotension, cardiac arrythmias, heart failure
- altered mental status seizures
- rhabdomyolysis
Causes of increased urinary excretion in hypo?
- Vitamin D deficiency
- Drugs e.g. diuretics,
- Primary Hyperparathyroidism
- Genetic/acquired syndromes of phosphate wasting
Symptoms of hypophophatemia?
- tetany
- weakness
- respiratory failure
- hypertension
- cardiac arrythmias
- heart failure
- altered mental status seizures
- rhabdomyolysis
