What does medical clerking involve?
Seeks to identify a patient’s problems based on physical examination and information obtained through conversation with the patient.
Checks the allergies / medications / drug history
What is the name of an outpatient prescription form from the GP?
FP10 = small green sheet of paper.
What is medicines reconciliation?
Process of creating most accurate list of all medicines the patient is taking and comparing this against the doctor’s prescription.
What does intentional omission mean?
For example = he had an illness and therefore, the doctor intentionally chooses not to prescribe the drug.
What does unintentional omission mean?
For example = the person does NOT remember the drug he has taken and therefore, the doctor does NOT prescribe it.
Who and when does the ward round where drug charts are reviewed ?
Doctors (junior and consultant) review everyday with ALL patients.
Who and when does the medical review where drug charts are reviewed ?
Doctors with patients who become MORE unwell.
Who and when does the drug round where drug charts are reviewed ?
Nurses with time intervals of 8:00 / 14:00 / 18:00 / 22:00
All patients are seen in order to administer medications.
Who and when does the pharmacist review where drug charts are reviewed ?
On admission with new patients.
On discharge with patients going home.
Every day with ALL patients.
What is a TTO?
This is known as a ‘To Take Out’ prescription and is prescribed by the doctor when the patients is leaving the hospital.
Checked / Supplied by the pharmacist.
What does the TTO document?
Documents:
- Allergies / Reaction.
- Admission medicines that remain the same.
- Medicines have been changed.
- Instructions to GP and Pharmacist.
- Document new medicines prescribed since admission.
How are medicines supplied in the hospital?
Stocks on the wards. Patient = From their own supply. Pharmacy. Ordered into the pharmacy. Nurse administers the medication. Medications need a prescription on the inpatient drug chart.
How are medicines supplied in the community?
General Sale
Pharmacy Only
Prescription Only = supplied by the pharmacy only.
How are prescription only medicines supplied in the community?
GP prescribes a prescription.
Community pharmacist supplies the medication and thus someone administers the medication.
