what is discharge planning
a process that aims to improve the coordination of services after discharge from hospital by considering the patients needs within the community. a process of identifying and overcoming potential challenges to a smooth transition into the community
who is involved in discharge planning
medical, SLT, dietician, physio, OT, patient/client, nursing, family, social work
key points for an effective discharge
involve the patient/client, start planning at admission stage, involve family and care givers, set goals, involves the whole MDT, early referrals to other proessions
discharge documentation/legislation
section 2- requires an NHS body to notify social services of a patients likely need for community care services after discharge (assess within 3 days), section 5- notifies social services of the proposed date of the patient discharge
Discharge options
home, RH, NH, rehabilitation unit, home with intermediate care/ESDS/ intermediate care in different setting, sheltered/warden controlled, family member house, hospice
information needed for a safe discharge- patient
physical ability- can they get OOB, transfer, walk, climb, stairs/steps, get washed, dressed, make their meals, carry food, get their shopping, do their laundry, drive, walk outside
cognitive ability- can they communicate/ understand how to wash, dress, make meals, shop. can they remember to take their medication?
information needed for a safe discharge- home environment
carers- do they live alone, what family and friends are available, are they available every day, what can they help with, are they physically and mentally stable
home- flat, bungalow, house, maisonette. Stairs, steps, outside toilet, bed/toilet downstairs. walk in shower, shower over bath. kitchen- gas, electricity. oven low or high
social support for patients
home care with things like cleaning and shopping, disability equipment and adaptations to your home, day centres to give you or the person who cares for you a break ,day care for child if either you or they are disabled, care homes
disability equipment and transment
OT assessment- chair raises, rails, kitchen equipment, bathroom, equipment, bed levers, stair lifts, ramps, widen doors, downstairs toilet, wheelchairs
mobility scooters, community alarm, bus passess, community transport services, garden maintenance service, meals on wheels, driving with disability
once patient is discharged
you might see them in: home setting, gym setting/private practice, OP setting
exercise classes
warm up, main exercises, cool down, equipement, emergency procedures, outcome measures, inclusion and exclusion criteria, education sessions
