Insertion: deep part to glute tuberosity of femur, superficial 2/3 of ITB
Innervation: L5-S2

Origin: inferomedial aspect of ischial tuberosity of of pelvis
Insertion: medial surface of proximal part of tibia (behind sart/grac)
Innervation: Sciatic (tibial) L5-S2

Origin: superolateral facet of ischial tuberosity
Insertion: posteromedial surface of medial tibial condyle
Innervation: Sciatic (tibial) L5-S2

Origins: Long head- inferomedial facet on ischial tuberosity
Short head- lower 1/2 lateral lip linea aspera of femur
Insertion: head of fibula
Innervation: Sciatic L5-S2
