Bleeding disorder caused by low factor VIII
Hemophilia A
Bleeding disorder caused by low factor IX
Hemophilia B
No spontaneous bleeding, delayed onset of bleeding after trauma surgery or extractions. 6-49% normal clotting factor
Mild hemophilia
Bleeding with minor trauma, severe bleeding with surgery 1-5% normal clotting factor
Moderate hemophilia
Spontaneous bleeding, excessive bleeding with trauma or surgery , <1% clotting factor
Severe hemophilia
Prolonged PTT
Normal PT
Normal platelet count
Normal bleeding time
Hemophilia
Post extraction treatment for all forms of hemophilia
Antifibrinolytic agents- tranecamic acid 25mg/kg 3 times daily for 5-7 days
Soft diet 7 days
Examine pt after 24-48 hours to check for bleeding
Considerations for hemophilia pts
Avoid block,IM, lingual infiltration, injection in floor of mouth
Avoid aspirin and NSAIDS
Mild to moderate hemophilia pts undergoing surg procedure
Use desmopressin, tranexamic acid or aminocaprioc acid
Severe hemophilia pt undergoing surgery
Needs factor VIII replacement
Absence of bon willebrand’s factor causes failure to form platelet plug
Prolonged PTT, prolonged bleeding time
Von Willebrand’s disease
