What drugs are given in HF? and why
Diuretic - fluid overload
ACE inhibitor - BP control
B blockers - reduce hr and force of contraction - allow proper filling. also by inhibiting sympathetic drive that can cause apoptosis of myocytes
Spironolactone - diuretic, retains K+ and unknown MOA acts as antifibrotic agent against metaloproteases.
Neutral Endopeptidase inhibitor
SGLT2 inhibitor - osmotic diuresis

Bisoprolol , metoprolol
e.g. of ACE inhibitor
Captopril, benazepril ( aka lotensin)

MOA of NEPs
Neutral endopeptidase inhibitors prevent breakdown of BNP + allow longer duration of action i.e. prolong the body’s own coping mechanism
How do ace inhibitors work?
Prevent conversion of ang1 to ang2 therefore decreases BP, stoops vasoconstriction
Important in HF because this reduces the pressure of the systemic system into which the LV has to pump blood
( also stop water retention -> lower bp, and prevents aldosterone production -> further prevention of diuretic action)
