A
Site- where is the pain?
Onset- did the pain come on gradually or instantly? Was it constant or fluctuating? How often? Any warnings of onset?
Character- describe the pain for me… sharp, dull, tight band
Radiation- eye, neck, face
Associated sx- nausea, vomiting, photophobia, dizzy, rash, mood/concentration changes, LOC, speech, seizures?
Timing- when do they usually occur
Exacerbating/Relieving- light/noises/foods
Severity 1-10 does it affect ur sleep?
