Warfarin

A
CI- pregnant
M-INR (once maintained every 3 months- should be between 2.5-3.5)
I-7As: 
Antibiotics
Analgesics
Antiplatelets
Alcohol
Antidepressants
Anti-pregnancy 
Amiodarone
Advice- never take 2 doses if one dose missed, dont use with NSAIDs and aspirin §

Low molecular weight heparins eg enoxaparin

A

Cautions- patients with renal impairment require dose adjustment
M- FBC, U&Es before treatment, creatine clearnace (shouldnt be <30ml/min)
I-NSAIDs, anticoagulants, antiplatelets
SE-LMWH Low platelets, More bleeding, Weird reactions at injection site, hyperkalaemia, hypersensitivity, Hair loss

Aspirin

A

CI- <16 years old
I- NSAIDs, anticoagulants, SSRIs
SE- ASPIRIN Asthma, Salicylism, Peptic ulcer and Premature closure of ductus arteriosus, Intestinal blood loss, Reyes syndrome, Indigestion, Noise (tinitus)
