Mild- amoxicillin

Severe- co-trimoxazole

Oral Metronidazole 400mg 8 hourly
severe or no improvement- oral vancomycin
Intraabdominal sepsis abx?
Gentamicin IV + metronidazole IV + Amoxicillin IV
Cellulitis abx?
Limited soft tissue infection- Oral flucloxacillin (or clarithromycin)
Moderate to severe cellulitits- flucloxacillin IV (allergy- vancomycin)
Sepsis of unknown source
Amoxicillin IV + Gentamicin IV ± metronidazole (penicillin allergy- gentamicin + vancomycin ± metronidazole
Neutropenic sepsis abx?
Piperacillin/ Tazobactam 4.5g 6hourly IV
allergy ceftrazidime IV
Meningitis abx?
Ceftriaxone 2g 12hourly IV
(allergy- chloramphenicol)
If >55 cover listeria + amoxicillin
+ dexamethasone IV for 4 days
Endocarditis abx?
Amoxicillin 2g IV 4 hourly (allergy- vancomycin)
± gentamicin IV

Ciprofloxacin
Clindamycin
