Temperature
Oral or tympanic
Radial Pulse
If regular, count for 30 seconds and multiply by 2
Respirations
Count while finger is still on the radial pulse for one minute
Blood Pressure
Both arms (perform in one arm)
Include measurement of pulse obliteration point to assess for ausculatory gap.
General Assessment
Assessing appearance, behavior, speech, articulation, language, mood, thought process and perceptions.

For color, vascularity, moisture, temperature, texture, thickness, mobility and turgor, presence of skin lesions.

Head, face, and scalp for size, shape, and symmetry

PALPATE:
Skull, scalp, and hair for distribution, condition, texture, tenderness, lesions

Eyes
INSPECT AND PALPATE:
Eyebrows, eyelashes, upper and lower lids, conjunctiva, sclera, corneas, lacrimal glands, pupils, iris, eye alignment

INSPECT:
External appearance, size, shape, deformities, position, lesions, discharge, edema, and erythema.

Noting color, translucency, lesions, landmarks - umbo, handle of the malleus, short process, cone of light, and TM mobility.

Lips, oral/buccal mucosa, gums and teeth, dentition, tongue, palate, sublingual region, salivary gland openings, tonsils, posterior pharyngeal wall.
