Temperature
Oral or tympanic
Radial Pulse
If regular, count for 30 seconds and multiply by 2
Respirations
Count while finger is still on the radial pulse for one minute
Blood Pressure
Both arms (perform in one arm)
Include measurement of pulse obliteration point to assess for ausculatory gap.
General Assessment
Assessing appearance, behavior, speech, articulation, language, mood, thought process and perceptions.

Wrist and Hand
Distal radius and ulna, wrist joint, anatomical snuffbox, carpal bones, metacarpals, phalanges.
Spine
INSPECT:
Patient’s posture, spinal curvatures.
PALPATE:
Spinous processes, SI joints, paravertebral muscles.
Hip
INSPECT:
Patient’s gait.
PALPATE:
Iliac crest, ASIS, greater trochanter.
Knee
Quadriceps muscle, patella, patellar tendon, femoral condyles, tibial plateau, tibial tuberosity, popliteal fossa.
Ankle and Foot
Medial and lateral malleolus, calcaneus, anterior ankle joint, achilles tendon, plantar fascia, MTP joints, heads of metatarsals.
Facial Nerve CN7
Assess facial movements at rest and during conversation.
Spinal Accessory Nerve CN 11
Strength and contraction of sternomastoid and upper trapezius muscles.
Motor
Observe body position during movement and at rest, noting any involuntary movements, muscle bulk, and tone.
