What is BPH?
Benign prostatic hypertrophy
What does BPH cause?
Irritation (urgency, frequency, nocturia)
Obstruction (decreased stream, incomplete emptying, straining)
How common is BPH?
> 50% of all men by 50 yo
What causes hemorrhoids?
Straining during bowel movements
Constipation
Sitting for long periods of time
Anal infections
Certain diseases like cirrhosis
When are hemorrhoids most common?
During pregnancy and after childbirth
Which anal sphincter is voluntary?
External anal sphincter
Which anal sphincter is involuntary?
Internal anal sphincter
Where is the prostate located relative to rectal structures?
In between the middle and inferior transverse rectal folds.
You can palpate the median sulcus and lateral lobe.
What are external hemorrhoids?
Dilated hemorrhoidal veins that originate below the pectinate line and are covered with skin.
What causes external hemorrhoids to actually hurt?
Thrombosis.
Note:
Will appear as a tender, swollen, bluish, ovoid mass.
What do internal hemorrhoids look like?
Enlargements of normal vasculature above the pectinate line.
Usually not palpable.
What do internal hemorrhoids cause when ruptured?
Bright-red bleeding during defecation.
What are prolapsed hemorrhoids?
Reddish, moist, protruding masses when a patient bears down.
What does a rectal prolapse look like?
A doughnut of red tissue.
What do I check during a DRE in men?
Prostate also at the 12oclock position.
