Inflammatory monoarthropathy

Commonly first metatarsophalygeal joint of big toe

Can be polyarticular

Attacks may be precipitated by trauma, surgery, starvation, infection, diuretics

High dose NSAID
If CI, use colchicine
Steroids may be used
Rest and levate joint

Prevention

A

Lose weight
Avoid prolonged fast
Avoid alcohol excess
Stop smoking

If >1 attack in 12m, tophi or renal stones:
Allopurinol - reduce plasma urate
Febuxostat - if allopurinol not rtolerated - decreases uric acid
