Describe the definition of Gout
Definition: gout is a form of acute inflammatory arthritis predominantly affecting the first metatarsal-phalangeal joint.
Describe the aetiology and pathophysiology of Gout
Pathophysiology: renal under-excretion causes excessive amount of uric acid within the blood, causing hyperuricemia in 90% of cases. This causes uric acid to crystallizes to form renal calculi deposited around joints or tendons. However gout predominantly affects the the metatarsal-phalangeal joint.
List the main signs and symptoms for Gout
- Inflammation causing red, hot, swollen joints within the 1st metatarsal. 2. Intermittent bouts of pain and flare ups. 3. DX by hematology testing where high erythrocyte sedimentation rate is confirmed by visualization of crystals.
Describe the responses/benefits of EXP for people w/Gout
- Improved metabolic function: decreased insulin resistance and enhanced kidney function. 2. Reduced risk of co-morbidities
Describe the contraindications to EXP for people w/Gout
Avoid ex during flare ups and wait till it is nearly/completely resolved
What are the recommendations and modifications for people w/Gout?
- Modification of diet; aim to reduce intake of meat, fish and alcohol. 2. Weight loss; to reduce risk of co-morbidities e.g. CVD
