Gout Flashcards
Acute gout management
NSAIDs (indomethacin)
glucocorticoids
colchicine
prophylactic urate lowering therapy
low dose colchicine, low dose NSAIDs or corticosteroids for atleast 6mo
Colchicine MoA
microtubule assembly inhibitor
colchicine toxicity
nausea
vomiting
abdominal pain
diarrhea
myopathy, neutropenia, aplastic anemia and alopecia in chronic use
chronic gout treatment strategies
a) xanthine oxidase inhibitors to reduce synthesis of uric acid
b) uricosuric drugs to increase uric acid excretion
xanthine oxidase inhibitors
Allopurinol
Febuxostat
Allopurinol toxicity
hypersensitivity reactions (skin rashes)
Allopurinol & Febuxostat drug reactions
interferes with the metabolism of 6-mercaptopurine, azathioprine and theophylline
uricosuric drug
Probenecid
Probenecid MoA
blocks proximal tubular reabsorption of uric acid
Probenecid drug reactions
inhibits the excretion of methotrexate, naproxen, ketoprofen & indomethacin
