- inhibit ach release
- increase tone and rhythmic contraction of intestine but decrease propulsion
- loperamide and codeine also antisecretory action- increase Na absorption and decrease cl secretion

• Prolonged fever shigella
• toxic megacolon C diff
• acute ulcerative colitis, antibiotic - associated colitis
Not recommended for children
