Ondansetron
When is ondansetron used?
Chemotherapy and radiation emesis
PONV
What is the mechanism of action of ondansetron?
Blocks both peripheral and central 5-HT3 receptors
5-HT from drugs results in action potentials which release transmitters to stimulate the NTS and CTz and this the VC
Ondansetron blocks the firing of these potentials
What is the results of 5HT 3 antagonists?
reduce ACUTE vomiting
less effective in the delayed phase
Which setron is better at preventing delayed emesis?
Palonosetron
When are HT antagonists not effective
Motion sickness or dopamergic trnasmission induced vomitng
What are the side effects of setrons
Headache
Constipation
Name a Neurokinin receptor antagonists
Aprepitant
What is the mechanism of NK1 receptors?
Block NK1 receptos in the VC and CTZ that can be stimulated by endogenous substances
When are NK1 blockers used
Late phase emesis and PONV

Prophylacxis and treatment of motion sickness
What is the mechanism of action of hysosine?
Block M acetylecholine receptors at mutlipe sites eg NTS and VC
Directly inhibit GI movements and relax GO tract
What are the side effects of hysosine?
Numerous eg blurred vision, urinayr retention, dry mouth, sedation
Name a Histamine H1 receptor antagonis used to treat emesis?
Cyclizine
When are H1 blockers used?
Motion sichness
Acute labyrinthitis
What is cyclizines mechanism of action?
Blockade of H1 receptors in vestibular nuclei and NST
What are the side effects of cyclizines?
Sedation and drowsiness
Name two dopamine receptor antagonists used in emesis?
Domperidone and metoclopramide
When are they used?
Drug induced vomiting - do not use in children
Not effective in motion sickness
What is the mechanism of action of dopamine receptor antagonists?
Centrally block dopamine D2 receptors in the CTZ
Peripherally exert a prokinetic action on the oesophagus, stomach and intestine
What are the side effects of domperidone and metoclopramide?
Domperidone does not cross the BBB and is less likely to result in the many unwanted effects of metoclopramide [e.g. disorders of movement (extrapyramidal effects)]
When is phenothiazine used?
Sever nausea and vomiting
