Acute Cholecystitis
Pain can radiate to right shoulder
Acute Appendicitis Symptoms
Acute periumbilical pain which starts to localize at McBurney’s point (RLQ)., anorexia, low-grade fever. + psoas and obturator signs
Acute Diverticulitis
S/sx: high fever, anorexia, n/v, LLQ pain
CBC includes Leukocytosis w/ neutrophilia, shift to the left
Acute Pancreatitis
+ Cullen’s sign (blue discoloration around umbilicus)
+ Grey Turner’s sign (blue discoloration on the flanks)
Signs: onset of fever & n/v w/ abd pain that radiates to the epigastric region and LLQ
C. Diff
Caused by gram-positive anaerobic bacterium
Antibiotics that contribute: Clinda, fluoroquinolones, cephalosporins, PCNs
Colon Ca screening
USPSTF recommends between 45-75 yo
Chrohn’s
Can affect the entire GI tract
+psoas/illiopsoas sign
Acute Appendicitis
RL! abd pain w/ passive right hip extension
+ obturator sign
Acute Appendicitis
internal rotation of the hip causes RLQ abd pain
+ Rovsing’s Sign
Acute Appendicitis
Deep palpation of LLQ results in referred pain to RLQ
McBurney’s Point
Acute Appendicitis
Max tenderness at 1.5-2 inches from the anterior superior iliac spine on a straight line to the umbilicus (in the RLQ)
+ Murphy’s Maneuver
Cholecystitis or gallbladder disease
Grey Turner’s Sign
+ pancreatitis
Ecchymosis or discoloration of flank, sign of intra-abdominal hemorrhage
