What would you do if you join a new ship and find LSA equipment damaged or missing?
Raise a non conformity as per the SMS, deal with the defect and/or ask the MCA to complete a general inspection and get the MCA’s written approval to sail.
If it is a critical piece of equipment - compass, oily water separator etc, raise a non conformity, inform port state, flag state, class, DPA and request dispensation if needing to sail before it can be repaired.
What is a classification society?
An organisation that establishes and applies technical standards in relation to the design, construction and survey of marine-related facilities including ships and offshore structures.

What is Ballast Water Management?
The Ballast Water Management Convention requires all ships in international trade to manage their ballast water and sediments, according to a ship-specific ballast water
management plan. All ships must carry a ballast water record book and an International Ballast Water Management Certificate.
The regulations apply to All UK vessels and any vessel in UK waters. They do not apply to:
- Vessels which are not designed to carry ballast water
- Ships which only operate in a single body of water
- Warships
- Ships which carry permanent ballast in sealed tanks
D-1 standard requires ships to exchange their ballast water in open seas, away from coastal waters - ideally >200nm
D-2 is a performance standard which specifies the maximum amount of viable organisms allowed to be discharged. All new ships are required to meet D-2 standard so will be fitted with special equipment to do so.
Logbook:
- When ballast is taken onboard
- When ballast is discharged overboard/to a reception facility
- When ballast water is treated or circulated
- Accidental uptake of ballast water
- Any other remarks
International Ballast Water Management Certificate:
- 400GT
- 5 years
- Annual, intermediate, renewal
Death onboard - how would you deal with it and who would you contact?
- Confirm they are dead
- Enter details into OLB
- Take photographs if required to be used as evidence
- Preserve body as per Ships Captains medical guide - stripped, body bag, stored 4°C
- Notify Register of Shipping and Seamen - required to produce a death certificate
- Notify next port health authority. Complete maritime declaration of health if not from natural causes
- Notify DPA, Flag etc
Discuss Crew accommodation, galley and food/water inspections
Crew Accommodation:
- Heating/air conditioning operational
- Adequate ventilation
- Adequate lighting
- Hot/cold running water
- Defects or damage
- General hygiene inspection
Entry made in OLB with date & time, signed by Master and crewmember making the inspections
Food & Drinking water:
- Food supplied are suitable in respect of quantity, nutritional value, quality and variety
- Takes into account different religous requirements
- Do not contain any food which is likely to cause sickness or injury to health
- Fit for consumption
Galley:
- All spaces including behind equipment should be easy to clean
- Furniture and fittings should be resistant to dirt and moisture
- Any metal should be rust proof
- Fridges and freezers clean. Temps 4°C for fridges, -21°C for freezers
How often should shower heads be cleaned?
Super chlorinated every 3 months, 50ppm
What is the Masters Discretion?
SOLAS regulation 34-1
The owner, the charterer, the company operating the ship or any other person shall not prevent or restrict the Master of the ship from taking or executing any decision which, in the Master’s professional judgement, is necessary for safety of life at sea and protection of the
marine environment.
There must be a written statement in the ships SMS highlighting this.
What is the purpose of the MAIB?
The Marine Accident Investigation Branch investigates marine casualties involving UK flagged ships, and foreign flagged ships in UK territorial waters. The purpose is to prevent futher avoidable accidents from recurring, not to apportion blame.
They are an independent body which operates under the Department for Transport
- Carrying out investigations to determine the causes of accidents at sea
- Publishing reports that include safety recommendations on improving safety at sea
- Increasing awareness of how marine accidentss happen
- Improving national and international cooperation in marine accident investigations

Examples of Marine Accidents
- Death or serious injury to a person - unable to perform duties for >72 hours or hospital for >24 hours
- Loss of a person from a ship
- Loss, presumed loss or abandonment of a ship
- Material damage to a ship
- Requires dispensation to proceed to sea
- Breakdown requiring towage
- Stranding or disabling at sea
- Collision
- Pollution
Examples of Marine Incidents
- Close quarter situations where urgent action was required to avoid collision
- Any event that had the potential to result in a serious injury
- A fire that did not result in material damage
- Unintended temporary grounding
- Person overboard who was recovered without serious injury
- Snagging of fishing gear resulting in a dangerous heel
