2. Bile stasis - impaired gallbladder emptying
What are the three types of gall stones?
- Cholesterol stones
- Black stones
- Mixed Brown pigment sones
which is the most common type of gall stone?
cholesterol stone
Name 4 major risk factors for cholesterol stones?
- Obesity
- Pregnancy /Female sex
- Family history
- Malabsorbtion
- crohns/cystic fibrosis
(6Fs - fat,female,fertile,fourty.family history)
cholesterol stones - pathophys
- abnormal hepatic cholesterole metabolism
- leads to high cholesterole concentration in hype
- hypersaturated - precipitation of cholesterole
black pigment stones - pathophys
- increased haemolysis
- increased uptake and conjugation of bilirubin
- black pigment stones form
black pigment stones - risk factors
- increased hemolysis
- increased unconjugated billirubin
- increased conjugation and bilirubin in bile
- high concentration of bilirubin in the bile causes black pigment stone
mixed/brown pigment stones - risk factors
bacterial infections and parasites
clinical features of gall stones
- most are asymptomatic
- Biliary colic
- constant dull RUQ pain
- lasts > 6 hours
- post prandial
- radiates to right shoulder and back
