what are the 2 generic risks of antifungals?

QT prolongation

lowest risk = fluconazole, highest in ketokonazole
can itraconzaole be taken with carbonated drinks?
yes - improves bioavailability
what should you do if someone on ketaconazole comes in with dark urine/N&V/abdo pain?
suspend treatment immediately, could be sign on hepatotoxicity
is ketoconazole an oral formulation?
no - been suspended, usually just in shampoo/topical
what antifungal can cause phototoxicty?
voriconazole
what 3 things do you need to consider in someone prescribed amphotericin B?
- caution in renal failure
- anaphylaxis risk - test dose with 30mins observation
- maintain same formulation between conventional, liposomal and lipid complex formulations (serious harm and fatal overdoses have occurred)
what prophylactic medication should you have on standby for someone being treated with amphotericin B?
antipyretics or hydrocortisone in patients with previous reactions
