C1-C3
1) Respiratory = ventilator dependent, can’t clear secretions
2) Bed mobility = total assist (full hospital bed)
3) Bed, wc transfers = total assist with transfer board or lift with swing
C4
1) Respiratory = may be able to breathe without a ventilator
2) Bed, wc transfers = total assist
3) Eating = total assist
C5
1) Respiratory = low endurance and vital capacity secondary to paralysis of intercostals; may require assist to clear secretions
2) Bed Mobility = some assist with full hospital bed in pts control, side rails
3) Eating = total assist for setup, then I eating with equipment
C6
1) Respiratory = same as C5
2) Bed, wc transfers = some assistance needed for level transfer
3) Eating = I w/ or w/o equipment (except cutting which is total assist)
C7-C8
1) Respiratory = low endurance and vital capacity
2) Bed mobility = I to some assist
3) Eating = independent
T1-9
1) Respiratory = compromised vital capacity and endurance
2) Transfers = independent
3) Standing/Ambulation = S is independent, A is typically not functional
T10-L1
1) Respiratory = intact respiratory function
2) Pressure relief = independent with wc cushion, postural support devices, pressure relief mattress
3) Transportation = independent in car (including loading of wc) - with hand controls
L2-S5
1) Bowel = independent with padded toilet seat
2) Pressure relief = independent with wc cushion, postural support as needed
3) Standing/Ambulation = Standing is I, A is functional (may need some assist)
