malignant prolif of lymphocytes in blood
many disorders, CLL in most important
only in adults
slow and incurable
CLL things to know
small, mature lymphocytes in blood
same thing as small lymphocytic lymphoma really.
***positive for b cell antigens (CD19,20,21), but also CD5 (weird)!!!!!!!!!!!! - this will be on the test
negative for TdT
older patients, asymptomatic at first, long, incurable
lab findings in CLL
WBC ^
hypogammaglobulinemia
genetic abnormalities of CLL
bcl-2 gene rearrangement
normal bcl-2 prevents apoptosis
they are essentially immortal
hairy cell leukemia
thinks to know
bigger, hairy cells that are usually seen most in marrow. they form chicken wire looking fibrosis, making it hard for bone marrow to leave.
splenomegaly - really huge spleens!! no lymphadenopathy…
pancytopenia! monocytopenia!

immunophenotype of hairy cell leukemia

positive for b-cell antigens, CD25!, and CD11c!
Negative for CD5 (that is in CLL)
prognosis of hairy cell?
good! often just need one course of chemo

rare
aggressive
large granulated lymphocyte leukemia things to know
large granulated lymphocytes - granules are very prominent and small
t-cell
neutropenia
long survival
