what is fibromyalgia?
Persistent widespread muscoloskeletal pain, stiffness, disrupted unrefreshing sleep, easy fatigability, paresthesia, accompanied by multiple painful tender points widely and symmetrically distributed
diagnosis
(1) a history of widespread pain lasting > 3 months
(2) pain (not just tenderness) in at least 11 of the 18 tender point sites on digital palpation
how do fibromyalgia pts present?
- pain all over
- fatigue
- poor sleep
- cognitive impairment
PE findings for fibromyalgia
Specific sites or points of pain—more tender or painful than the same sites in normal persons
how to dx fibromyalgia?
- avoid excessive tests
- empathy
- exercise
- reduce stress
- SSRI/SNRI/NSAIDs
main meds for fibromyalgia
Milnacipran (Savella)
Duloxetine (Cymbalta)
what are some anti-convulsants
gabapentin (Neurontin)
tiagabine (Gabitril)
pregabalin (Lyrica)
