Immunocompetent - serology CMV IgM and IgG (IgG low avidity in priamry infecton)
Immunocompromised - Serology - limited diagnostic value
Heterophile’s Ab’s - Paul Bunnel/monospot - clumping of sheep RBCs
Treatment of CMV
Gancivlovir (valganciclovir - prodrug), Cidofovir, Foscarnet
Roseola virus
Roseola infantum (exanthum subitum, Sixth disease)
3 days fever, then transient rash
Abx prescribed for fever - rash blamed on penicillin allergy
Most common cause of febrile convulsions
Latent in mono/lymphocytes
Can cause pneumonitis, hepatitis, encephalitis in BMT
Diagnosis - BLood PCR
Rx - Ganciclovir, foscarnet or cidofivir
Infectious mononucleosis - Triad
EBV Fever Pharyngitis Lymphadenopathy Incubation - 4-6 weeks Maculopapular rash

Predisposes to lymphoma
Treatment - reduce immunosuppression + give rituximab (anti-CD20 monoclonal Ab)
EBV is not dangerous in pregnancy

Genitally transmitted ASsociated with Kaposi's sarcoma PRimary effusion lymphoma Castleman's disease (non-cancerous growth in LNs) Rx - Ganciclovir and FOscarnet
