Fetal environment registry review Flashcards
Amniotic fluid index (AFI)
Dividing uterus into 4 quadrants and measuring the largest pocket of fluid in each quadrant (exclude fetal parts/cord) and add together
-Progressively increases until 28 weeks then slows
Normal AFI range
5-24cm
Deepest vertical pocket (DVP)
Maximum vertical pocket excluding fetal parts and cord
Oligohydramnios
AFI <5cm / DVP <2cm
Associated with: demise, renal, IUGR, PROM, post dates
Polyhydramnios
AFI >24cm / DVP >8cm
Associated with: GI atresia, skeletal, open defects, hydrops, cystic hygroma
Biophysical profile
Total possible 8/8
-Fetal breathing: 30 secs (2)
-Gross fetal movement: 3 movements (2)
-Fetal tone: 1 limb/hand felxion or extension (2)
-Amniotic fluid: 1 pocket measuring 2cm x1cm (2)
*NO movement = loss points for movement and tone
Lung maturity
2nd to 3rd trimester = MOST accurate measure by amniocentisis for L/S ratio (lecithin/sphingomyelin)
Sonographic evidence of lung maturity
Lung increasingly echogenic compared to liver in 3rd trimester
