Febrile disease patterns Flashcards

1
Q

Shan Han Disease Pattern

A

Follows 6 channel (comportment, level) theory:
Taiyang SI/UB greater yang
Yangming LI/ST bright yang
Shoayang SJ/GB lesser yang
Taiyin LU/SP greater yin

Progression of 6 - level patterns in “days”

A

1st day: taiyang invaded: headache, back rigidness
2nd day: yangming invaded: body has fever (4 big)
3rd day: shaoyang invaded: chest hypochondriac pain, hearing loss
4th day: taiyin invaded: abdominal distention, dry throat
5th day: shaoyin invaded: dry dry dry
6th day: jueyin invaded, irritability contraction of scrotum, menstrual/fertility issues

Exterior Febrile Disease Patterns

A
  1. Tai Yang:
    a) Wind-cold=shang ha: no sweating, body aches, P: tight
    b) Wind-strike=zong fen, fever + sweating
    c) Organ= H20 in UB, edema, urinary sxs.
  2. Wen Bing: Wind-Warm- fever>cold, streppy, swollen glands, yellow d/c
  3. wei qi:
    a) Damp-warm= damp percolating, heavy, fatigue, glomus in chest, ST Duct oppression, unsurfaced heat, P: soggy
    b) Autumn dryness= only in Autumn

Aversion to cold/fever, but dryness path., Dry Dry Dry

Q

HALF/HALF-febrile

(membrane level)

;

A
  1. Shaoyang = alt. hot/cold, bitter taste, eye dizziness, wiry pulse, (LV signs bec GB)
  2. Malarial =chills→fever→sweating→normal
    a) Hot= (more severe) more heat, heat signs >cold signs, delirium
    b) Vacuity= More qi/bld def signs, low energy, greater damage to system

Distinction betw malarial and shaoyang = Malarial = days (long time ) of fever, days (long time) off fever. Shaoyang more rapid cycling

Heat at Blood Aspect

A

Heat at blood aspect:

  • vigorous night fever
  • macules on skin
  • vomiting blood
  • bloody stools
  • reckless blood

T: crimson

P: fine and rapid

17
Q

Interior Patterns- febrile

Exuberant Heat Stirring Wind

A

Exuberant Heat Stirring Wind: (ying/blood level)

  • vigorous heat+ LV wind signs dizziness distention and pain agitation arched back
18
Q

Internal Patterns- febrile

Heat Entering Construction Aspect

A

Heat entering construction aspect=ying level

  • heat at night
  • heart vexation
  • delirium
  • dry throat

T: crimson, no coat

P: fine, rapid

19
Q

Internal patterns-febrile

Vacuities

A

deficient heat

YIN= mylar flush, night sweats

Blood= Pale, yellow face, pale lips. White, dry fur

Qi= fever stronger after exertion, pale face, spont. sweating, fatigue

Yang= (false heat) cold interior, heat signs exerior

20
Q

Interior patterns-febrile

Infantile summer heat

A

Infantile summer heat=summer only

  • scorching, tidal fever
  • fever for 2-3 months+thirst
  • no sweating profuse urination

can be after birth or w/ abscesses

21
Q

Types of Heat

A

Mild heat- PT feels it; dr may not

Aversion to heat-lying in cool spot, no clothes/covers

Heat vexation-HT vexation; agitation

steaming bone-internal; not felt on surface

Tidal fever-comes and goes at specific time

Unsurfaced fever-heat rises w/ lengthy touch

scorching fever-hot to touch

vigorous fever- heat obvious, high body temp

fulminant fever-sudden

22
Q

four levels in a nutshell

A

defensive qi: aversion to cold+fever (external)

QI: fever, feeling heat (no aversion to cold) (4 bigs)

Ying level: mental changes, fever @ night (yin deficiency)

Blood: fever @night, bleeding, macules, wind (ungrounded blood)

23
Q

Three Burner Theory

A
  1. Upper Jiao:
    a) Heat evil in LU=exterior wind warm, bound stools,

T: red, yellow or blood flecked coat

b) Heat in PC= ying/jueyin level, cold limbs, delirium, T: red, stiff, no coat
2. Middle Jiao: Digestion= “hot, stuck, crazy”
a) Heat in yangming- fever worse in afternoon, 4 bigs
b) Damp- heat in SP- epigastric fullness
3. Lower Jiao= blood level, like shaoyin, Jue yin, UB/LI dzs
a) Heat in KD like yon def, dry throat, deafness, T: deep red, no coat
b) LV heat stirs wind: high fever at night, coma, convulsions T: deep red, no coat
c) LV empty wind= dry black teeth, dry cracked lips, convulsions, limb tremors. T: crimson/no coat

Lower Jiao

Q

6 Levels

A
  1. Taiyang channel:

Shang ha

Zhong Fen

warm wind (Wen bing)

  1. Yangming:

a) 4 bigs T: red, coating yellow dry, P: flooding, big, forceful

b) organ: tidal fever afternoon, 5 palm sweat, distention/pain in abdomen/umbilicous, constipation. T: coating is thick yellow, brown, black, with cracks, P: deep, strong, rapid/slow

  1. Shaoyang: ½ ½ alternating chills and fever, bitter taste, dry throat, blurred vision, fullness, distention, hypochondriac region, low appetite, nausea, vomitting irritabililty T: coating tin and white or sl yellow P: wiry
  2. Taiyin: abdominal fullness, vomiting, diarrhea, low appetite, no thirst, fluctuating abd. pain, better w/ pressure and warmth T: white, greasy coating P: deep, moderate, weak
  3. Shaoyin:

a) Han Hua (cold) aversion to cold, no fever, minute pulse, lie down, faint pulse, deficient shen, cold extremeties, diarrhea w/ undigested food, vomit after eating, red cheeks (false heat)

b) Re Hua (Heat) very irritable, cannot lie down, dry mouth and throat, insomnia, T: red w/ scanty yeallow cioat, P: thin, rapid (deficient heat0
6. Jue yin: wasting and thirsting, qi rising to HT, pain in HT, hinger - no desire to eat, may vomit parasites,, mixed heat and cold pattern
