Sou = phlegm
Ke sou= cough
Chuan= panting (SOB)
Xiao = wheezing, phlegm rales
Rales= clicking, rattling, cracking noises

Cough characteristics

A

Always involve LU
Loud= excess
Soft= deficiency
Dry= dryness (excess) or LU Yin Xu

LUs govern qi (heavenly qi- that wc we breath; earth qi- that which we eat).
Take heavenly qi turn into what we can use
govern exhale.
Depurative

(SP stores phlegm in LU, can disrupt flow of qi- cause cough- why PTs need to take care of SP)

KD initiates inhale, grasps the qi, pulls it down=little discussed respiratory organ

*inhale v exhale not that useful diagnostically

A

Enduring: months/years
Night: something to do w/ esophagus
Fifth watch: before sunrise, phlegm
Dry cough: >10x before phlegm comes out, dry throat
Wheezing cough: xiao (asthmatic)
Mute cough (hoarse/no sound)
Long bout cough/whooping cough (pertussis) woop on inhale on last cough

A

must occur in Autumn
Phlegm reduced= dry cough, harder to expectorate, might be streaks of blood,
LU loaths dryness/likes moisture
dry mouth, thirst, dry nose
slants toward warm dry or cold dry
challenge: moisten LU w/o creating phlegm

Interior (no chills, aversion to cold/wind), Fifth watch or night cough,
White phlegm (thick or thin), cool limbs, pale face, lack of thirst, clear, long urination

A

Pure EXCESS pattern
HOT phlegm
Loud cough
hyperactivity, panting/rough breathing, fever, no chills
Bronchitis, thick, smelly, yellow or green phlegm, maybe blood streaked,
hurts chest to cough, rib side pain, thirst, reddish urine, bound stools
T: red w/ yellow fur
P: big, forceful, rapid

A

Dryness is external Pathogen,
dry cough w/ phlegm difficult to expectorate like strands of flour,
May be blood streaked

cough more severe with pain

P: thin (even tho is excess- causes yin def), rapid

Phlegm lines (bubbly/white) on tongue, sticky, greasy thick, slimy

A

suspended Rheum= upper jiao, H2O stuck in rib sides=SOB, panting, pain
spillage rheum= edema limbs
propping rheum= accumulation of H2O in abdomen, chronic, can last years-wei qi/yin def & yang def

floating edema=face
sinking edema= limbs

SP Yang compromised, Maybe KI too

KI root of SP= sloppy wet stools, UFP, sore back/knees

counterflow panting= LU supposed to bring down-instead wheezing (audible breathing), cough & qi going up.
Dyspnea=SOB
1) asthma- wheezing component-phlegm involved

A

LU Qi Xu- spontaneous sweating

LU Yin Xu-hasty panting, malar flush, dry, day cough, thirst

KI Yang Xu-panting, exhale greater than inhale, SOB- worse w/ exertion, Prob LU Qi def too, ancillary of muscles of neck used minimal forceless pulse in 3rd pos

KI Yin Xu- looks like Jing Def: forgetfulness, insomnia, 5-palm heat, night heat, tidal fevers
T: Red, dry peeled, can be deep red (deng says purple)

Xiao Chuan Misc Patterns

A
  1. Pregnancy: SOB, fatigue, diaphragm cant move down. If complains, check for blood def.
  2. throat wind (Hou Feng):
    a) Acute throat wind: sudden/anaphylactic
    b) Putrefying throat wind- ulceration of throat/infection/HSV
    c) throat locking wind- clenched jaws/epileptic seizure, panting
    d) throat entwining wind/rigid neck
  3. Diptheria- white, patchy spot, cough like dog barking
