Ke Sou vocablualry
Ke= sound Sou = phlegm Ke sou= cough Chuan= panting (SOB) Xiao = wheezing, phlegm rales Rales= clicking, rattling, cracking noises
Cough characteristics
Always involve LU
Loud= excess
Soft= deficiency
Dry= dryness (excess) or LU Yin Xu
LUs govern qi (heavenly qi- that wc we breath; earth qi- that which we eat).
Take heavenly qi turn into what we can use
govern exhale.
Depurative
(SP stores phlegm in LU, can disrupt flow of qi- cause cough- why PTs need to take care of SP)
KD initiates inhale, grasps the qi, pulls it down=little discussed respiratory organ
*inhale v exhale not that useful diagnostically
Cough types
Enduring: months/years
Night: something to do w/ esophagus
Fifth watch: before sunrise, phlegm
Dry cough: >10x before phlegm comes out, dry throat
Wheezing cough: xiao (asthmatic)
Mute cough (hoarse/no sound)
Long bout cough/whooping cough (pertussis) woop on inhale on last cough

Phlegm reduced= dry cough, harder to expectorate, might be streaks of blood,
LU loaths dryness/likes moisture
dry mouth, thirst, dry nose
slants toward warm dry or cold dry
challenge: moisten LU w/o creating phlegm

Heavy turbid cough
Interior (no chills, aversion to cold/wind), Fifth watch or night cough,
White phlegm (thick or thin), cool limbs, pale face, lack of thirst, clear, long urination
T: white tongue coat
P: slow
Heat Evil Congesting LU
Pure EXCESS pattern
HOT phlegm
Loud cough
hyperactivity, panting/rough breathing, fever, no chills
Bronchitis, thick, smelly, yellow or green phlegm, maybe blood streaked,
hurts chest to cough, rib side pain, thirst, reddish urine, bound stools
T: red w/ yellow fur
P: big, forceful, rapid
Summer heat
(must occur during summer-heat period)
Damage dry cough w/o phlegm or scanty frothy sputum, rapid breathing, pain in chest, vigorous fever, vexing thirst, profuse sweat, head ache, dizziness, fatigued limbs and spirit fatigue
P: surging/rapid
Dryness Evil Damages LU
Dryness is external Pathogen,
dry cough w/ phlegm difficult to expectorate like strands of flour,
May be blood streaked
panting w/ white frothy sputum,

Diptheria
barking cough white sticky membrane in back of throat cannot take it off hoarseness foul odor in mouth blue/green lips palpitations; throat swells up 20% of kids under 5 die 5% fatality in adults
Measles
fever
sticky white spots in mouth (sesame seeds)
skin rash
4 Cs: cough, Coplis’s spots, conjunctivitis (tearing of eyes), Coryza (nasal inflammation)

Dry, thirst + dry throat
enduring (long term), phlegm a little bit blood streaked

cold= junky cough, wheezing, profuse white phlegm, heaviness in chest, glomus feeling

Phlegm lines (bubbly/white) on tongue, sticky, greasy thick, slimy
Rheum types
suspended Rheum= upper jiao, H2O stuck in rib sides=SOB, panting, pain
spillage rheum= edema limbs
propping rheum= accumulation of H2O in abdomen, chronic, can last years-wei qi/yin def & yang def
floating edema=face
sinking edema= limbs
Water cold invades LU
spillage Rheum: cough, deficient copious, thin white frothy phlegm edema in limbs/ankles-water getting stuck in tissue- not TnTing, COPD, emphazema SP Yang compromised, Maybe KI too scanty urine (KI yang not working) Fluids stag and affect HT=palpitations KI root of SP= sloppy wet stools, UFP, sore back/knees cold (likes covers)
Propping Rheum
propping up bec otherwise PT cant beath water below HT long standing Chest fullness, SOB profuse, white, frothy phlegm Edema of face and head dizziness (water forced to head) sooty black complexion T; White, slimy tongue fur P: taught, wiry, string like body under distress- makes HT tight

Blood Stasis Cough
HT/PC/LU comes with dark, purple blood foul smell long term festering stag in upper jiao gargle but not drink SOB palpitations, pain/oppression in chest/ HT disease pattern (send to DR)
T: Purple, macules
P: bound, choppy, regularly irregular= Dai Mai pulse
Panting (Xiao Chuan)
Hasty Panting = fast SOB
counterflow panting= LU supposed to bring down-instead wheezing (audible breathing), cough & qi going up.
Dyspnea=SOB
1) asthma- wheezing component-phlegm involved
2) frog rail throat

can be hot or cold

panting turbid long-standing copious, difficult to expectorate phlegm fullness, oppression in chest heavy limbs, fatigue sometimes cough so severe, people vomit expect SP, QI Def signs/mixed excess& def T: white, shiny P: slippery
Deficient Xiao Chuan Patterns (panting)
LU Qi Xu- spontaneous sweating
LU Yin Xu-hasty panting, malar flush, dry, day cough, thirst
KI Yang Xu-panting, exhale greater than inhale, SOB- worse w/ exertion, Prob LU Qi def too, ancillary of muscles of neck used minimal forceless pulse in 3rd pos
KI Yin Xu- looks like Jing Def: forgetfulness, insomnia, 5-palm heat, night heat, tidal fevers
T: Red, dry peeled, can be deep red (deng says purple)

Xiao Chuan Misc Patterns
- Pregnancy: SOB, fatigue, diaphragm cant move down. If complains, check for blood def.
- throat wind (Hou Feng):
a) Acute throat wind: sudden/anaphylactic
b) Putrefying throat wind- ulceration of throat/infection/HSV
c) throat locking wind- clenched jaws/epileptic seizure, panting
d) throat entwining wind/rigid neck - Diptheria- white, patchy spot, cough like dog barking
