O: lesser wing of sphenoid bone, anterosuperior optic canal
I: tarsal plate and skin of upper eyelid
A: elevates upper eyelids
I: CN III (oculomotor nerve)
Superior rectus
O: common tendinous ring
I: sclera just posterior to cornea
A: elevates, adducts and rotates medially
I: CN III (oculomotor nerve)
Inferior rectus
O: common tendinous ring
I: anterior sclera
A: depresses, adducts and rotates medially
I: CN III (oculomotor nerve)
Medial rectus
O: common tendinous ring
I: anterior sclera
A: adducts eyeball
I: CN III (oculomotor nerve)
Lateral rectus
O: common tendinous ring
I: anterior sclera
A: abducts eyeball
I: CN VI (abducens nerve)
Superior oblique
O: body of sphenoid bone above optic canal
I: passes through a trochlea and inserts into sclera
A: medially rotates, depresses, and abducts eyeball
I: CN IV (trochlear nerve)
Inferior oblique
O: floor of orbit
I: sclera deep to lateral rectus
A: laterally rotates, elevates, and abducts eyeball
I: CN III (oculomotor nerve)
