What are considerations for stroke?
- avoid Valsalva
- begin walking at slow speed and consider body weight support
- attend to mood and cognition
- anticipate early onset of local and general fatigue
Exercise goals for stroke:
Aerobic: inc cardiovascular function, functional abilityl–reduce CV risk
Resistance: inc functional capacity, reduce risk of falls
Flexibility: inc ROM/prevent contracture
What is PD?
progressive loss of dopaminergic cells in basal ganglia
Signs of PD:
rigidity, resting tremor, bradykinesia, gait and postural disturbances, cognitive and affective disturbances
What are considerations for PD?
- assess balance, gait, mobility prior to ex testing
- measure HR and BP for autonomic dys
- thermoregulation & orthostatic hypotension
- DBSs interfere with ECG signal
- emphasize cervical and trunk rotation/mobility
Side effects of medications for PD:
dyskinesia, dysrhythmia, brady/tachycardia, orthostatic hypotension, elevated metabolic rate, depression/anxiety, hallucination, impaired/absent sweating
What is important to remember with resistance training and PD?
avoid free weights for safety reasons
What is MS?
chronic, inflammatory demyelination of CNS that disrupts neural transmission
What are considerations for MS?
spasticity, dyscoordination, fatigue, cardiovascular dysautonomia, visual/cognitive disturbances, heat sensitivity, medications, agonist/antagonist imbalance, hydration, incontinence, exacerbation/remission
What is the Uhthoff phenomenon?
transient (<24hr) exacerbation of neurological symptoms associated with exercise or elevated body temp
Motor neuron diseases spare with type of neurons?
sensory, duh
What are considerations for MND?
weakness, atrophy, spasticity, dyscoordination, fatigue, depression, cognitive disturbances (laugh/cry inappropriately)
What is the recommended prescription of exercise for MND?
none, they are progressive diseases, so person will never recover, but may be able to function better and inc QOL
