equine athlete Flashcards
Exercise induced pulmonary haemorrhage
Risk factors of EIPH
speed
light jockets
previous eiph
number of years in training
hard groudn/bar shoes
cold external temps
upper airway disorders
impact of Grade 1 and 2 EIPH
associated with normal pefromance
clinical presentation of EIPH
usually seen in racehorses within 4 hrs of intese exercise
-bursts of speed+/-breath holding
bilateral epistaxis
poor performance
caudodorsal lungfield
impact of grade 3&4 EIPH
associated with poor performance
diagnosing EIPH
ce
tracheal endoscopy <2 hrs post race–>presence of blood, grading
BAL<14 days post race–>RBCs, haemsiderophages, low grade inflammation
imaging
treating EIPH
SUpportive care
rest
anti-inflammatories as blood in airways cause inflammation
EIPH causes fibrosis
intrapulmonary blood porvokes macrophage influx and activation
-these inflammatory cells results in reversible disruption of alveolar septal architecture
-chronic macrophage activity coincides with development of alveolar septal wall thickening and fibrosis
-will likely cause permanent alterations to alveolar blood air barrier and reduce local pulmonary compliance
managing EIPH
furosemide 4 hrs prior to intense exercise
-not allowed on day of racing in uk
-rest after episode
