Define epilepsy
A conditions where a person is subject to recurrent seizures
At least 2 unprovoked seizures occurring more than 25 hours apart
Factors predisposing a person to recurrent seizures x5
- Focal seizures
- Neurological deficits
- Mental retardation
- Structural brain disease
- Psychiatric and behavioral disorders
Define a seizure
Paroxysmal hypersynchronous discharge of cerebral cortical neurones without inhibition
When is epilepsy considered to have resolved
Seizure free for more than 10 years, with no medication for the past 5 years
Differential diagnosis for epilepsy x5
- Syncope
- Pseudo seizures
- Metabolic disturbances
- Cardiac arrhythmias
- Mental disturbances
Treatment of seizures- 1st and 2nd line
1st- carbamazepine, Na valproate
2nd- Levetiracetum, Pregabaline
Non pharmacological treatment x2
- Surgery to remove affected lobe
- Neuro-stimulation- vagus nerve, deep brain
Triad of partial seizures
- Aura
- Automatisms
- Post ictal confusion
3 methods involved in pathogenesis of epilepsy
- Excessive excitability of neurons > hypersynchrony
- Cellular processes - altered activity of voltage gated channels > altered resting potential
- Increased central role of excitatory neurotransmitter
Infective cause of focal lesions x5
- TB
- Brain abscess
- Toxoplasmosis
- Neurocysticercosis
- Progressive multifocal leukoencephalopathy
