1= adenocarcinoma 2= uterine serous and clear cell carcinoma
What is the precursor to endometrial adenocarcinoma?
Hyperplasia with atypia
What are some risk factors for endometrial hyperplasia?
High circulating/unopposed oestrogen- tamoxifen, obesity, PCOS
What is the presentation of endometrial hyperplasia?
Abnormal vaginal bleeding
How is endometrial hyperplasia diagnosed?
Transvaginal US- thickened endometrium- >4mm are menopause
Biopsy
What is the management of endometrial hyperplasia?
No atypia= High dose progestogens
Atypia= Hysterectomy
What is the precursor to uterine serious and clear cell carcinoma?
Serous intraepithelial carcinoma
What is the more common type of endometrial cancer?
Adenocarcinoma
What is the more aggressive type of endometrial cancer?
Uterine serous and clear cell carcinoma
Who is uterine serous and clear cell carcinoma seen in?
Generally older ladies
What are the risk factors for endometrial carcinoma?
Post menopausal
High circulating oestrogen- PCIS, obesity, early menarche, late menopause
Unopposed oestrogen
HNPCC familial cancer syndrome
What is the presentation of endometrial cancer?
Cardinal symptom= PMB
Abnormal bleeding
Uterine mass, fixed uterus
How is endometrial cancer diagnosed?
Transvaginal US
Biopsy
What is the management of endometrial cancer?
TAH and BSO with peritoneal washings
What is the management of high risk endometrial cancer?
TAH and BSO with peritoneal washings and chemo
