What is the biggest etiological factor with cervical cancer?
> 90% HPV 16 and 18 related
What is the mechanism by which HPV causes atypia?
Viral DNA integrates into host cell genome, leading to over expression of viral proteins and dysregulation of host cell cycle
What are the risk factors for cervical cancer?
Multiple partners Early age at first intercourse Older age of partner Smoking Age 45-49
What is the presentation of cerical cancer?
Abnormal bleeding- PCB, IMB, PMB
Discharge- watery, mucoid, purulent, malodorous
Rarely pain
What is the screening for cervical cancer?
Every 3 years from 25-49 and every 5 from 50-64
Smear test for high risk HPV and/or cytology
If pregnant, arrange for 12 weeks post delivery
How is cervical cancer diagnosed?
Biopsy
Imaging- MRI, CT
How is cervical intraepithelial neoplasia managed?
LLETZ excision
Thermal coagulation
Laser ablation
How is cervical cancer managed?
Excision
Hysterectomy
Chemoradiotherapy
What is the prognosis for cervical cancer?
Relatively good
