Why are the elderly at risk?
Greater risks for ADR because of homeostasis changes. multiple chronic conditions, OTC meds, polypharmacy, inability to take drugs correctly, or health literacy.
Absorption- dyspnea and reduced blood flow
Disruption- higher body fat
Metabolism - less effective
Excretion - May have renal impairment
Elderly people are also more sensitive to sedation, and may have altered immunity or frality.
Why are renal/liver diease patients at risk?
The primary route of excretion for drugs is the urine, via the kidneys. The PH of the urine and diuresis can affect this. This can be a risk, as the patient may have failed to excrete drugs or metabolise them. They may have increased sensitivity to drugs and their side effects.
Dose size may be reduced, and increase dose intervals.
Avoid drugs with a narrow therapeutic window.s
