Regularity: Irregular Rate: 60-100 P wave: Normal and upright, one in front of each QRS PRI: .12 - .20 seconds QRS < .12 seconds
Sinus arrhythmia
Regularity: Regular Rate: 150-250 P wave: Abnormal: coming from different foci in heart PRI: .12-.20 seconds QRS: < .12 seconds
Atrial tachycardia
Regularity: Can be regular or irregular Rate: 250-350 bpm (of atrium) P wave: “Saw Tooth” appearance PRI: Not measurable QRS: Normal
Atrial flutter
Regularity: Irregularly, irregular Rate: >350 P wave: Not measurable PRI: Not Measurable QRS: Normal
Atrial fibrillation
Regularity: Regular
Rate: 40-60 bpm
P wave: Inverted (in Lead II) before, during, or after QRS
PRI: Measurable only if before the QRS, then < 0.12
Junctional rhythm
Regularity: Regular
Rate: 61-100 bpm
P wave: Inverted (in Lead II) before, during, or after QRS
PRI: Measurable only if before the QRS, then 0.12
Accelerated junctional rhythm
Regularity: Regular
Rate: 101-180 bpm
P wave: Inverted (in Lead II) before, during, or after QRS
PRI: Measurable only if before the QRS, then < 0.12
Junctional tachycardia
Regularity: Usually irregular because of PJC
Rate: Depends on the underlying rhythm
P wave: Inverted before, during, or after QRS
PRI: Measurable only if before the QRS, then < 0.12
Premature junctional contractions
a broad term for a group of rhythms originating above the ventricle

Supraventricular tachycardia (SVT)
Regularity: Irregular Rate: Depends on underlying rhythm P wave: PVC does not have P wave PRI: PVC (no PRI); Underlying rhythm (PRI Normal) QRS: PVC wide bizarre, others normal
Premature ventricular contraction
Regularity: Usually regular Rate: 140-180 bpm (or more) P wave: None PRI: None QRS: Wide bizarre, >.12 sec
Ventricular tachycardia
Regularity: Usually regular Rate: 20-40 bpm P wave: None PRI: None QRS: Wide bizarre, >.12 sec
Idioventricular rhythm
Delay at AV node. Each atrial impulse is eventually conducted through ventricle.
PRI > .20 seconds; constant
First degree heart block
Regularity: Irregular in a pattern of grouped beating
Rate: Usually slightly slower than normal
P wave: Upright and uniform: some P waves are not followed by QRS
PRI: Progressively lengthens until one P wave is blocked
QRS:
Type I second degree heart block (Wenckebach) (Mobitz I)
Regularity: P-P interval is regular, R-R interval can be regular or irregular
Rate: Usually in the bradycardia range
P wave: Upright and uniform, more than one P wave for each QRS
PRI: Always constant, can be greater than .20
QRS: < 0.12 seconds
Type II second degree heart block (Mobitz II)
Regularity: Irregular but can be regular
Rate: Atrial rate usually normal (60-100)
Ventricular rate 40-60 if focus junctional
20-40 if focus is ventricular
P wave: Upright and uniform; more P waves than QRS complexes
PRI: No relationship between P waves and QRS
QRS: < 0.12 seconds if focus junctional, > 0.12 if ventricular
Complete heart block/third degree
