Dysmenorrhoea is characterised by excessive pain during the menstrual period.

WHAT IS PRIMARY DYSMENORRHOEA?
No underlying pelvic pathology
affects up to 50% of menstruating women
usually with in 1-2 years of menarche
Cause of primary dysmenorrhea?
excessive endometrial prostaglandin production
what are the features of primary dysmenorrhea?
pain typically starts just before or within a few hours of the period starting
suprapubic cramping pains which may radiate to the back or down the thigh
What is the management of primary dymenorrhoea?
1ST LINE: NSAIDs - mefanamic acid ND IBUROFEN - THEY INHIBIT PROSTAGLANDIN PRODUCTION
2ND LINE: COCP
What is secondary dysmenorrhea?
develops after many years of menarche
an underlying pathology is the cause
starts 3-4 days before onset of period
causes of secondary dysmenorrhea?
endometriosis
adenomyosis
pelvic inflammatory disease
intrauterine devices* Cu coil
fibroids
