NSAIDS/Salicylates
1st line for self treatment
Mechanism: inhibition of prostaglandin biosynthesis: cox inhibition
ANALGESIC, ANTIPYRETIC, ANTI-INFLA
cox 1
normal cells/tissues
expressed in the stomach (protective)
Cox 2
induced with inflammation
not expressed in the stomach
NSAID adverse reactions
avoid with CV disease/ risk in the elderly
renal insuffiiency/failure
Na/H2O retention/ inc BP
interstitial nephritis
GI bleeding - if cox 1 inhib
Pregnancy - premature closing of ductus arteriosus
Liver toxicity signs
flu like Sx
>24 hours - liver pain, renal failure, liver inc
>72 hours jaudice
change in mentation
Acetaminophen
TYLENOL; inhibits CNS cyclooxygenase central inhibition of prostaglandins
NOT ANTI-INFLAMMATORY
watch out for OD because in so many OTC drugs
Spasmolytics
Skeletal Musc Relaxants
alleviate elevated muscle tone
WATCH OUT: sparing in elderly and CND effects/alcohol effects; some have addictive potential (timmy = must have nicotine)
Opiods
Agonist or antagonist with morphine-like activity
agonist - analgesia; no max dose; useful without major side effects
antagonist - reversal of agonist
Morphne
Gold Standard Opiod agonist low bioavailability (25%) half life 3 hrs - always taking it
Opiod Adverse Rxn
respiratory depression* constipation - needs to be addressed with LTerm use nausea/vomiting tolerance neuropsych myoclonus
Fentanyl patch
VERY potent - lasts 72 hours
NOT for acute pain
heating pad will inc _____rate = BAD DONT DO IT
Antidepressants/anticonvulsants
chronic - neuropathic pain –> reg NT
